3. Please indicate which puppy you are interested in or if you would like to be on a waiting list for a future litter. Puppies are numbered on this site. (Puppy #1, Girl #1, etc)

4. Are you familiar with the general disposition/temperament of the Standard Poodle or Labrador Retriever breed?


5. Are you familiar with the grooming needs of a Standard Poodle? (Only if applying for a poodle)
6. Have you owned or currently own a dog? A poodle? A Lab?



7. Are you planning to show and/or breed this puppy?

8. Will you be sure to keep your pup current on all shots, wormings, vet checks, etc.?
   
9. Are you willing to sign a spay/neuter contract if buying puppy for a pet/companion only?
 


 
10. Do you own your home or live in an apartment?
 


 
  If a home: do you have a securely fenced yard?
 



  If an apartment: do you have landlord's permission to have a large dog?
 



 
11. If you don't have a fenced yard, will you see that your pup gets ample exercise, such as regular walks and/or romps at the park?
 


 

12. Are you willing to teach your puppy basic training skills or enroll in a basic training class?

 


 
  Will you be crate training your puppy or another form of potty training?
 


 
13. Are you willing to purchase a high quality puppy/dog food which has meat or a meat meal (such as chicken meal, turkey meal etc.) as the first ingredient?
 


 

14. Will you love your puppy and welcome him/her into your family and home and promise never to abuse him/her?
